Shocked neighbors reportedly watched an Italian man tossing his brother's head into the street after decapitating him.
Brothers Benito, 57, and Annibale, 70, lived together in a Pannarano home, roughly 25 miles outside of Naples.
Police say it happened Wednesday night near Benevento south of Naples.
The killing took place after a fight over petty issues, police told ANSA.
Annibale went to bed and Benito allegedly grabbed a kitchen knife and attacked his brother.
Benito's confessed to the Carabinieri, Italy's national police. They found Annibale's headless body covered in stab wounds, according to the Daily Mail.
After questioning Benito overnight, authorities transferred him to a prison in Benevento.
The Public Prosecutor's Office in Benevento has launched a formal investigation and assigned a coroner to autopsy Annibale's body.
Italian media outlets have reported that the brothers were unhappy living together after Annibale's return from Rome, following his recent death of his wife.
